Aortic intramural haematoma: current therapeutic strategy.

چکیده

O ver the last 10 years, intramural haematoma (IMH) of the aorta has become increasingly recognised as a pathological entity distinct from aortic dissection. Despite this enhanced appreciation and the resultant increase in the frequency of its diagnostic identification, a consensus regarding optimal management strategies for this disease has not been established. This is due in part to our incomplete understanding of a condition only first clearly established in the mid 1980s. As opposed to aortic dissection where we have developed a relatively comprehensive knowledge of the more common constellations of anatomic involvement and their clinical outcomes, with IMH we are now only at a stage of correlating insights gleaned from clinical series contributed by investigators from around the world. Many of theses observations have been presented in the prior discussions.

Rapid diagnosis and management of thoracic aortic dissection and intramural haematoma: a prospective study of advantages of multiplane vs. biplane transoesophageal echocardiography.

AIMS The purposes of this study were to compare the accuracy of multiplane vs. biplane transoesophageal echocardiography (TEE) in the diagnosis of aortic dissection and aortic intramural haematoma, and to test whether these techniques provide all the diagnostic information required to make management decisions. METHODS AND RESULTS Fifty-eight consecutive patients with clinically suspected aor...

New vascular ring connectors in surgery for intramural haematoma of the abdominal aorta progressing to rupture.

Aortic intramural haematoma is similar to classic aortic dissection, which causes a life-threatening medical condition, and immediate diagnosis and treatment are crucial. The optimal therapy for intramural haematoma of the abdominal aorta remains controversial. Conservative medical management is usually the first choice of treatment for uncomplicated cases. Surgical intervention is usually requ...

Aortic intramural haematoma: natural history and predictive factors for complications.

I ntramural haematoma (IMH) of the aorta is attracting growing interest as a variant of aortic dissection and is more frequently diagnosed by modern tomographic imaging modalities in the evaluation of acute aortic syndromes. 2 The evolution from IMH to overt dissection or even rupture may occur suddenly or is heralded by ongoing acute aortic (pain) syndrome. Unlike classic aortic dissection, IM...

Extensive small bowel intramural haematoma secondary to warfarin

Intramural haematoma is a rare complication of oral anticoagulant therapy, occurring in  1 in 2500 patients treated with warfarin. This report describes a 71-year-old gentleman who presented with tachycardia, vomiting and abdominal distension on a background of anticoagulation for a metallic aortic valve. He was found to have a supratherapeutic international normalized ratio (INR) of 9.9 with a...

Aortic intramural haematoma: remarks and conclusions.

A ortic intramural haematoma (IMH) originates from spontaneous rupture of the aorta vasavasorum or from a penetrating atherosclerotic ulcer. Recent advances in imaging techniques have significantly aided its diagnosis, placing the incidence between 10–30% of acute aortic syndromes. Clinical presentation may be identical to that of classical dissection. IMH affects patients with more atheroscler...
